NOTICE OF APPEAL FORM:
The Board's standard Notice of Appeal form is now available for download in PDF format. The form cannot, at this time, be filled out or submitted online, but must be completed and mailed to the Board.

In order to view and print the form, you must have the free Adobe Acrobat Reader for Windows, DOS or for Macintosh installed on your computer. If you do not already have this software, click on the icon below and follow the instructions to install the Adobe Acrobat Reader on your computer.

APPOINTING AUTHORITIES:
The ADM 4055 (Order of Removal, Reduction, Suspension, Fine, Involuntary Disability Separation) form can now be filled out online! To view and print the form in PDF format, click here. Please note that Adobe Acrobat Reader does not allow you to save your completed forms; you must completely fill in and print the forms in one operation.
