In order to file an appeal with the State Personnel Board of Review, an individual must provide SPBR with written information indicating: the individual's name, address and telephone number; the name, address and telephone number of the individual's employer; and a general outline of the action being appealed.
Appellants are required to notify the Board in writing of any change in their address during the pendency of their appeal.
There is no specific form or document on which appeals must be submitted, but you may use the Board's standard Appeal Notice if you wish -- click here to access the form in PDF format. In order to view and print the Appeal Notice, you must install free Adobe Acrobat Reader software. Please visit get.adobe.com/reader for more information and to download the software.
Ohio Administrative Code 124-1-02(J) and 124-11-09 allow documents to be filed electronically with the State Personnel Board of Review.
If you file materials electronically, please do not file an additional paper copy.
